413 Kidney Stones [27 March 2017]


Kidney stones is a painful condition when crystals, usually calcium oxalate but also uric acid and other types, form in the kidneys. Small stones pass through without problems but larger ones can lodge in the urinary tract and block the flow of urine causing severe pain.

About 1 in 10 Canadians experience kidney stones at some time. Men are much more likely than women to develop kidney stones.

There is much you can do to prevent the formation of kidney stones:
• Drink 2-3 litres of water throughout the day to keep urine pale yellow
• Avoid soda pop as the phosphoric acid promotes stone formation
• Limit consumption of sugar, especially fructose
• Limit protein to 1g / 2 lbs lean body weight
• Limit red meat which lowers your citrate level
• Avoid or limit high oxalate content foods: beets, rhubarb, spinach, Swiss chard, sweet potatoes
• Avoid excess sodium
• Take extra magnesium, at least 1:1 with calcium
• Eat calcium-rich foods but go easy on calcium supplements – calcium in foods binds oxalates in the intestine preventing their absorption
• Maintain a healthy weight – obesity is a risk factor for kidney stones
• Exercise reduces risk of kidney stones

Stones that are too large to pass comfortably on their own require medical attention. Sound waves can be used to break up larger stones or surgery may be required. Potassium citrate is sometimes given to alkalize the urine and dissolve calcium oxalate stones but has some unpleasant side effects. Recent research using hydroxycitrate, an extract from the tamarind fruit (Garcinia cambogia) is showing promise of being a more effective and safer alternative.

364 Soda Pop [4 April 2016]


“Sugar-sweetened soda consumption is consistently associated with an increased risk of several chronic inflammatory diseases such as type 2 diabetes and cardiovascular diseases” begins the abstract to a 2014 study on soda consumption and rheumatoid arthritis (RA).

This study followed nearly 200,000 women from two Nurses’ Health Studies spanning nearly four decades totaling 3,381,268 person years. Information on soda consumption was obtained from a food-frequency questionnaire at baseline and approximately every four years there-after. During the study period 857 cases of RA were documented. After adjusting for known variables, the researchers found that women who consumed one or more servings of sugar sweetened soda per day had a 63% greater risk of developing seropositive RA than those who drank none. For later onset (after age 55) RA the risk was even greater. No association was found for seronegative RA or for diet soda consumption.

Similarly a 2014 systematic review and meta-analysis of five studies examined the associations of sugar-sweetened and artificially sweetened soda with chronic kidney disease (CKD). They found statistically significant increased risks of CKD with sugar-sweetened soda consumption but not with artificially sweetened soda.

But fruit juice and artificially sweetened beverages aren’t much better when it comes to risk of Type II Diabetes (T2D) and weight gain. A 2015 systematic review and meta-analysis concluded that while sugar sweetened beverages increased the risk of T2D (as would be expected), so also did fruit juice and artificially sweetened beverages, although to a lesser extent. Another study found that fruit juice with added sugar, but not 100% fruit juice, was associated with increased risk of T2D. A 2015 U. of Texas study found that seniors who consumed diet soda over 10 years increased their waist size (belly fat) by 70 to 500% more than those who didn’t. Other studies have found an association between diet soda consumption and risk of metabolic syndrome and diabetes.

Consumption of sweetened beverages is a deceptive way to add calories to your diet. A 1 liter bottle of Cola has 108 grams (27 teaspoons) of sugar with 400 calories. It would take 5 large apples to equal that in sugar. Next to quitting smoking, eliminating soda and other sweetened beverages could be the best thing you could do for your health.

333 Preventing Alzheimer’s [24 August 2015]


We often joke about developing dementia: You can hide your own Easter eggs or Bad news: “you have Alzheimer’s”; good news: “you can go home and forget about it”. There is a poem about aging with these lines:
I've got used to my arthritis, to dentures I'm resigned,
I can manage my bifocals, but oh how I miss my mind.
Seriously, most people would rather lose their vision, hearing or ability to walk than lose their mind. So what can we do to reduce our risk of developing dementia?

• Reduce sugar. High sugar consumption leads to insulin resistance including at the blood-brain barrier. This prevents insulin from reaching the brain and starves your brain cells of glucose. We can compensate with ketones from medium chain triglycerides such as from coconut oil, but prevention is better.
• Avoid diabetes. Diagnosis of diabetes doubles your risk of Alzheimer’s. Even mild elevation of blood sugar (100-110) significantly increases the risk.
• Consume enough good fats (including some saturated fat). A study from the Mayo Clinic found that a high carb diet was associated with an 89% increased risk for dementia while a high fat diet was associated with a 44% reduced risk.
• Keep your weight down. Obesity is an important risk factor.
• Exercise. Aerobic exercise has been found to reduce the risk of getting Alzheimer’s and also to improve brain function in those already diagnosed. Exercise reduces the amyloid protein associated with Alzheimer’s and improves memory.
• Don’t smoke and avoid excessive use of alcohol.
• Control blood pressure. High blood pressure is another risk factor for dementia.
• Get enough quality sleep. The brain’s waste removal system only works during deep sleep when the brain is flushed with cerebral spinal fluid which removes the amyloid plaques in brain tissue.
• Stay mentally and socially active. “Use it or lose it” applies to our brains as well as muscles & bones. Playing games like Scrabble, chess or bridge helps to keep the brain sharp and keeps you connected to people.

While none of these are guaranteed to prevent Alzheimer’s, following as many as you can should significantly increase your chances of keeping all your marbles to the end.

327 Leptin Resistance [13 July 2015]


I have previously written several times about the role of insulin and insulin resistance in weight gain but there is another compound involved called leptin.

Leptin, sometimes called the “satiety hormone”, was discovered in 1994. It is secreted by fat cells and tells our brain that we have enough stored energy. It reduces our appetite and promotes the burning of fat for energy. Leptin resistance occurs when this message is blocked, resulting in food cravings, continued fat storage, and eventually obesity. Leptin resistance can also be a factor in female infertility.

Several factors contribute to leptin resistance:
• excess dietary sugar, especially fructose
• high insulin levels
• high triglyceride levels
• inflammation
• MSG (SHN #135)
• lack of sleep
• stress
• low calorie diets that are not ketogenic (your body thinks it’s starving)

So what can we do to reverse leptin resistance?
• greatly reduce simple carbs and processed foods
• eat adequate protein and healthful fats
• avoid between meal snacks to give your liver a rest
• exercise (but don’t overdo cardio)
• get at least 8 hours of restful sleep
• lower insulin levels (e.g. with a ketogenic diet)

Leptin resistance is recognized as a key risk factor for obesity (Zhou & Rui, 2013). Identifying the causes and taking steps to reverse it will make any weight loss program easier to follow and much more effective. The ketogenic diet we use in our Rosetown weight loss clinic doesn’t specifically address leptin resistance but does indirectly deal with the factors causing it, particularly insulin levels, triglycerides, inflammation and dietary sugar.

321 D-Mannose & Bladder Infections [1 June 2015]


Bladder infections are a common problem for many women (and a few men). In most cases the best treatment is not antibiotics but sugar. A specific type of sugar called D-mannose, to be precise.

Most bladder infections (85-90% by one estimate) are caused by the bacterium E. coli which has an attraction for the sugar D-mannose. This particular sugar, which is a stereo-isomer of glucose, naturally occurs on the lining of the bladder. The E. coli bacteria stick to this sugar and thus to the bladder, preventing it from being washed away with urine flow. The solution is to give the bacteria what it wants. Taking a spoonful of this sugar every 3-4 hours for a day or two floods the bladder with D-mannose which collects the E. coli and flushes them away.

In cases where the infection has reached the kidneys or is caused by a different species of bacteria, D-mannose will be ineffective and antibiotics will be required. Antibiotics of course have the undesirable side effect of killing off beneficial bacteria as well as the pathogens, which as I discussed last week are what keeps us healthy. Trying D-mannose first is not only safer and more effective for E. coli infections, but also saves the antibiotics for when they are really needed. D-mannose does not affect intestinal bacteria and is safe for diabetics.

D-mannnose is found in several fruits such as peaches, apples, blueberries and especially cranberries. It is believed that this is why cranberry juice has a long and successful history of use for bladder infections. In one study (Prodromos et al, 1968) 73% of 44 women and 16 men with a bladder infection obtained relief with 16 oz of cranberry juice daily. The problem with cranberry juice is that most brands have sugar added and even the unsweetened juices still have a high fructose content. Taking cranberry concentrate capsules avoids the sugar, but none of the brands I carry list their D-mannose content on the label so you don’t know how much you are getting. Cranberries (and cranberry extract capsules) have many other nutritional components including vitamins, minerals, antioxidants, flavonoids, and polyphenolic compounds, so are beneficial for more than just bladder infections.

256 Sugar & Heart Disease [24 February 2014]

Did you catch the article on sugar in last week’s Sunday Phoenix newspaper? It reported on a recently published study from the Centers for Disease Control and Prevention (CDC) looking at sugar consumption and deaths from cardiovascular disease (CVD).

The researchers followed approximately 11,000 American adults for nearly 15 years, measured their sugar consumption, and recorded their deaths from CVD. The results were statistically adjusted for known risk factors of CVD like age, sex, ethnicity, smoking, etc. Compared to those who consumed less than 10% of their calories as added sugar, the increased risk of death from CVD was 30% for those who consumed 10 – 25%, and 175% for those who consumed over 25%. The increased risk of 175% means that you are nearly three times as likely to have a fatal heart attack. This finding was independent of sugar’s contribution to obesity which is itself a risk factor for CVD, so applies to “skinny” as well as overweight people.

What does this mean in terms of our diet? The World Health Organization recommends we get less than 10% of our calories from added sugar. Americans consume on average 15.5%; Canadians are somewhat better at 10.7%. Average of course means that half of us consume more than 10% and half of us less. Which half are you in?

According to the Mayo Clinic calorie calculator for a “somewhat active” man of 200 lbs, I need about 2500 calories a day. Ten % of this would be 250 which translates into 67 grams or 15 teaspoons of sugar. I could get over 15 teaspoons from just one can of pop (6-8), a chocolate bar (5-11) and a chocolate chip muffin (4-5). And these are the obvious sources – sugar is also added to ketchup, cereals, and even some yogurt. Learn to read the labels! Remember, if you are shorter or less active than I am, your 10% would be less. And that 10% should be a maximum – there is no lower limit for sugar consumption. Less (sugar) is definitely more (health).

230 Agave Syrup [19 August 2013]

Agave syrup (also called “agave nectar”) is marketed as a healthy natural sweetener. I have some for sale in my store. But just what is agave syrup and how healthy is it?

The syrup is made from the agave plant which grows in Mexico and southwestern USA (and from which Tequila is made). The best quality syrup is made from the leaf sap which is processed at a low temperature using enzymes and chemicals. Most syrup, however, is synthesized from the starch in the root with a similar process that converts cornstarch to high fructose corn syrup (HFCS). Agave syrup comes in two main grades: light syrup which is more highly filtered and has a mild flavor, and the darker colored amber syrup which has a stronger flavor. Agave syrup has become quite popular as a sweetener because of its flavor, sweetness, and perceived health benefits. A strong selling point is its low glycemic index, which is a measure of its effect on blood glucose levels.

But how healthy is it? It has more calories than table sugar but is sweeter so you can use less. It is highly processed so has few trace nutrients, unlike raw honey. But the biggest problem with agave is the amount of fructose it contains, from 60-90 % (compare with high fructose corn syrup at 55%). It is the fructose content that creates the low glycemic index often touted as a benefit. I have written before about the dangers of fructose [#55 22 March 2012]. It increases uric acid which causes gout; it contributes more to insulin resistance and obesity than other sugars; and it has a toxic effect on the liver [“Fructose, weight gain, and the insulin resistance syndrome” Elliott et al, 2002, JAClinNutr].

Dr Joseph Mercola recommends people in good health consume less than 25g of fructose per day (about 1 tablespoon of the syrup). He also advises those with gout, diabetes or insulin resistance to avoid it completely. From what I’ve read, I tend to agree with Mercola, and suggest that agave syrup be used sparingly by healthy people only.

142 Soda Pop [November 28, 2011]

Do you drink soda pop? If yes, then quitting may be the best single thing you can do to improve your health – maybe even better than quitting smoking. Here’s why.

The sugar content of most sodas is about 30 teaspoons in a 1L bottle. Most of the sugar in soda is in the form of high fructose corn syrup – the cheapest but also the most deadly (see my column #55 March 22, 2010). High sugar consumption is linked to obesity, high blood pressure, diabetes, liver & kidney disease, high triglycerides, vascular heart disease, and aggressive anti-social behavior. One quarter of Americans now consume 180 lbs or more of added sugar a year (that’s a pound every two days).

Diet sodas aren’t the answer either. There are other health problems with the synthetic sweeteners used in soda (see my column #14 June 1, 2009). The flavors are all synthetic chemicals with absolutely no nutritive value and questionable safety. Carbonated beverages removes oxygen from your blood and the phosphoric or citric acids leach calcium from your bones and teeth.
When you think about it, there is really no good reason for drinking pop and plenty for avoiding it. Your body needs plenty of pure water, it does not need sugar and artificial flavors. If you were offered poison and told that it tastes good and that drinking it would make you “cool” or popular, would you? Probably not. How many million dollars of advertising would it take to change your mind?

The principle of moderation may apply here. My wife Donna remembers as a child getting a small bottle of Orange Crush at the spring sports day once a year. I might drink a can or two of ginger ale a month. More than one a week however could be jeopardizing our health. Make the switch from drinking pop to water and discover how your health will improve. It may not be easy to quit though – I’m told that certain pops can be quite addicting. It will be worth it though because when it comes to beverages, water is still the real thing!
